North Olmsted Athletic Families,
Starting with the 2026-2027 school year, North Olmsted City Schools Athletic Department has partnered with FinalForms, an online forms and data management service to manage athletic forms and information. FinalForms will allow families to complete and sign athletic participation forms for your students. Information collected by FinalForms will be saved from season-to-season and year-to-year, meaning that you will never need to enter the same information multiple times. FinalForms will also pre-populate information wherever possible, for each of your students, saving you time. Families can review their forms at any time to verify it is current. You will be required to sign the athletic forms once per year and after any update.
Parents/guardians of all students planning to play a sport this year are required to create accounts in FinalForms and register your student(s). Please click HERE and follow the prompts to create your account, create your students and sign your forms. We have also included the Parent Playbook in our Forms menu, a step by step guide on how to get started in FinalForms.
